About This Course
Upon successful completion of the course, the students will be able to:
• discuss the changing demographic and health care trends in Singapore;
• explore evidence-based research in relation to healthcare and examine various research utilization models for application in clinical practice;
• identify various type of healthcare services and financial system available in supporting the needs of patients with chronic conditions;
• describe the theoretical knowledge of advanced pathophysiology related to chronic diseases and clinical management;
• discuss the therapeutic effects and adverse effects of the drug; and use of counselling skills to educate patients on compliance to medications;
• obtain a comprehensive history taking and physical examination with the correct technique and to interpret any abnormal clinical results and physical signs presented by the patients;
• apply health teaching strategies to educate patients to self-manage their chronic
• conditions and minimize long- term complications;
• apply motivational interviewing strategies and techniques to facilitate and engage intrinsic motivation within the patients and/or their families in order to elicit sustainable behaviour change;
• incorporate the nursing and case management processes to coordinate care and recognise how to access and utilise resources and networks to meet patient/family needs;
• demonstrate understanding of the principles of rehabilitation to achieve maximum functional independence for the patients;
• demonstrate understanding of the goals and principles that underpin the concept of palliative care nursing; and
• describe the physical, psychological, social and spiritual issues of patients in palliative care for those with non-cancer organ failures

Entry Requirements
- A local Polytechnic Diploma/ Degree in Health Sciences (Nursing) or equivalent
- Be a Registered Nurse with valid practicing certificate from the Singapore Nursing Board (SNB); and
- Have at least 2 years of clinical nursing practice; and
- Have a minimum of 1 year practice in the nursing specialty – nursing practice related to nursing management or care of patients with chronic diseases
- Successful applicants will need to undergo a compulsory medical examination and vaccinations before semester starts.
