Advanced Integrated Care Planning
About This Course
This module aims to equip students with the knowledge and skills to develop comprehensive care plans to effectively utilise community resources for care coordination and co-manage patients with chronic diseases in the community. It also introduces students to the principles of palliative care for advance care planning
What You'll Learn
Integrated transitional care helps to plan healthcare along systems and disciplines in multiple environments with a goal of making healthcare users’ experience personalised, flexible and seamless.
Entry Requirements
a. Relevant local Polytechnic Diploma or University Degree of any discipline related to healthcare; or
b. Relevant academic qualification includes Diplomas or Degrees in any other fields, and
• have relevant working experience in healthcare for at least 2 years; and/or
• obtained the Certificate in Transitional Care from SingHealth Academy.
Applicants who do not meet the entry requirements may be considered for admission to the course based on evidence of at least five (5) years of relevant working experience or supporting evidence of competency readiness. Suitable applicants who are shortlisted will have to go through an interview and/or entrance test.
