Advanced Care Coordination and Navigation
About This Course
Students will gain an appreciation of various financial schemes and community resources available for the patients, families and caregivers. Students will develop plans to coordinate with the multidisciplinary team of health professionals such as physical therapist and occupational therapist in the promotion of home rehabilitation.

This module provides students an overview of the dynamics of advanced care integration and how it achieves the best care outcome for clients.
Entry Requirements
a. Relevant local Polytechnic Diploma or University Degree of any discipline related to healthcare; or
b. Relevant academic qualification includes Diplomas or Degrees in any other fields, and
• have relevant working experience in healthcare for at least 2 years; and/or
• obtained the Certificate in Transitional Care from SingHealth Academy.
Applicants who do not meet the entry requirements may be considered for admission to the course based on evidence of at least five (5) years of relevant working experience or supporting evidence of competency readiness. Suitable applicants who are shortlisted will have to go through an interview and/or entrance test.
