About This Course
Upon completion of the course, students should be able to:
1. Describe patient’s journey through acute illness and transition processes between care settings
2. Perform patient assessment, which includes individual history-taking
3. Draft pharmaceutical care plans, through performing pharmacotherapy work-up and identifying patient-specific care goals and plan
4. Communicate (verbal/written) care plan to patient and/or recommendations to health care professionals
5. Perform necessary documentation of patient encounter and care plan
6. Conduct clinical case presentations
What You'll Learn
Entry Requirements
The candidate must have / be:
(1) Bachelor of Science in Pharmacy (with or without Honours classification) or an equivalent qualification
(2) Registered pharmacists with the Singapore Pharmacy Council (or overseas equivalent), and holding a current and valid practicing certificate
(3) at least 2 years of working experience as a patient-care pharmacist post pre-registration training (preferably)
(4) Completion of PR5134 Advanced Skills in Pharmacy Practice or equivalent training/work experience
